What do I bring?
If you have a referral from your doctor or other information from your doctor, please bring it with you as well.
If this appointment is for a work related injury and you are claiming through Workers Compensation Board you will need your WSIB claim number, your Social Insurance Number and your doctor’s referral.
If this appointment has to do with a motor vehicle accident you will need to bring the the following:
- Private claim form (Including policy and claim numbers)
- Your car insurance information (including claim number, adjusters name and contact number)
- Doctor’s Referral
- It will be helpful if you wear or bring appropriate clothing for the area you are having assessed/treated (shorts, tanktop, runners).
What are the costs?
Please contact us at (905) 778-9959 or visit us our location in Bradford to review the services we provide and their associated costs. Please note that for all WSIB claims, we will be requesting a credit card number to hold on record in the event that your claim is refused and WSIB does not pay for your treatment. If your claim is refused you will be responsible for all costs associated with your visits.
What if I can no longer attend my appointment?
Please note that you are billed for the time that you have scheduled with your therapist as they book that time specifically for you. If you are unable to attend and cancel your appointment with less than 24 hours notice you will be charged the full amount of your appointment. There is a 24 hour answering service (905-778-9959) where you can leave a message.
